②    Examination & Approval form for degree (2 copies)

③    Appointment letter for defense committee （Unlimited number of copies）

④    Recording paper for defense (Unlimited number of copies）

Come to the office, hand in the Examination & Approval form for degree (2 copies) for stamp and to take the ballots for defense.

Ⅱ. Before May 20th, student has to finish defense. After then, hand in the forms below.

①    Defense application form (1 copy) with stamp

②    Examination & Approval form for degree (2 copies)

③    Statement of authorization of the thesis (1 copy)

④    Thesis

⑤    Approval Form of Graduation for post graduate (4 copies)

⑥    Summary form for master student (1 copy)  signed by supervisor personally, A4 paper duplex print(print in both side of the paper)

⑦    Ballots, Thesis review form, Recording paper for defense

⑧    Photo- a one-inch color photo

For all the forms, please delete the English notes then print out!!
